SLIME LAB PARTICIPATION WAIVER, ASSUMPTION OF RISK, AND RELEASE OF LIABILITY

PLEASE READ CAREFULLY. THIS AGREEMENT AFFECTS YOUR LEGAL RIGHTS.

1. SIGNER INFORMATION

You must be at least 18 years old to complete and sign this waiver.

2. MINOR CHILDREN — OPTIONAL

By adding a minor, I certify that I am the minor's parent or legal guardian and have legal authority to sign for them. Every participating adult must sign their own waiver.

3. RISKS, SAFETY & EMERGENCY

Slime Lab activities involve slime and craft materials that may present risks including slips/falls, irritation or allergic reactions, choking or accidental ingestion, and damage to clothing or personal property. Slime and craft materials are not food. I am responsible for determining whether participation is appropriate for myself and any listed minor, disclosing relevant allergies or medical conditions, and following staff safety instructions.

Parents/guardians are responsible for minors during regular visits. Minors may be left without a parent/guardian only during an official Slime Lab drop-off program. In an emergency, I authorize Slime Lab to contact 911 and provide reasonable emergency assistance.

4. ASSUMPTION OF RISK & RELEASE OF LIABILITY

I voluntarily assume the inherent and reasonably foreseeable risks of participation.

TO THE FULLEST EXTENT PERMITTED BY CALIFORNIA LAW, I release and waive claims against The Fun Factory Lab LLC D/B/A Slime Lab and its owners, members, managers, employees, agents, contractors, affiliates, landlords, and property owners arising from participation, INCLUDING CLAIMS ARISING FROM THEIR ORDINARY NEGLIGENCE.

This release does not apply to gross negligence, reckless or willful misconduct, intentional wrongdoing, or liability that cannot legally be waived.

5. PHOTO/VIDEO CONSENT & TERM

I authorize Slime Lab to photograph or video me and any listed minor(s) and use the content for its website, social media, advertising, promotional materials, and in-store displays without compensation. This waiver remains effective for one year from the date signed.

6. SIGNATURE

BY SIGNING BELOW, I CONFIRM THAT I AM AT LEAST 18, HAVE READ AND VOLUNTARILY AGREE TO THIS ENTIRE WAIVER, INCLUDING THE LIABILITY RELEASE AND PHOTO/VIDEO CONSENT. IF I LISTED MINORS, I CERTIFY THAT I AM THEIR PARENT OR LEGAL GUARDIAN AND HAVE AUTHORITY TO SIGN FOR THEM.

By electronically signing and submitting this waiver, I agree that my electronic signature has the same effect as my handwritten signature.
